Referrals for Diagnostic Evaluations & Requirements

 If you do not have a diagnosis yet, start here. If you are looking to start services scroll down and complete the steps (intake form and intake packet) to get on our wait list for ABA therapy. 


Discuss Concerns with Your Doctor and Get an Evaluation

Have your child evaluated by a doctor and/or psychologist. You may or may not need a referral to a specialist. If your child receives a diagnosis, then you can move on to the next step.

Diagnostic evaluations must be conducted within the past 3 years, have a minimum of 2 assessment tools, a summary of each individual assessment, and must include the test scores (Autism Spectrum Disorder Services Policy, Chap. 801, pg. 14). Please remember that it can take 6 months-1 year to be able to get an appointment for a psychological evaluation, so we encourage you to make an appointment as early as possible. It is recommended that the provider complete the CARS or GARS and ADOS assessment at a minimum. 

Determine the right therapies for your child

Create a plan of care. Will your child be also receiving speech or OT? Now is also a great time to call your insurance and discuss benefits. 

Ready for Therapy?

If you already have a psychological evaluation and referral, then you are ready for the intake process.

Step 1. Intake Form & Intake Packet

Click below to fill out our contact form and we will give you a call to schedule your appointment should we have a spot open. Please be ready to upload your child's most recent psychological evaluation, referral, and a copy of your insurance card. We must have these on file to request authorization for assessment. We will also need you to fill out the intake packet below.

We will need both the intake form and the packet filled out to be able to move forward.

Upon completion, please email it to casey@spectrumga.com. If you have any trouble, feel free to stop by. We can print a copy for you to fill out or call or text Casey at 912-432-6748, and we will email a copy to you to fill out in which it will automatically send a copy to you and to us upon completion. We cannot move forward without this packet, as it gives us permission to request an assessment from your insurance.

Step 2. Request Insurance Authorization

Spectrum works with several insurance companies and we will request authorization for ABA assessment. 

Step 3. ABA Assessment

 We will complete a thorough ABA evaluation and craft a plan specifically for your child.

Step 4. Start Therapy

Once the plan is authorized, treatment may begin. Your child is assessed every 6 months and their plan is updated. Their progress is evaluated to determine the need for continued services or if they are ready to transition.
